RN must have experience with HCHB EMR and OASIS assessment visits. Territory covers Jacksonville and surrounding areas, up to 50 miles from the office Home Health RN Job Details: Pay Rate: $1,800-$2,000 Schedule: 8AM-5PM Available Shift: Days, Monday-Friday, 5 shifts per week Start Date: 12/1/25 Located In: Jacksonville, FL Requirements: Qualified applicants MUST have at least one year of experience in a Medicare-certified home health agency and be comfortable with OASIS (Outcome and Assessment Information Set), have a valid RN license and be willing to obtain FL licensure. As a Home Health Registered Nurse (RN), you’ll have the opportunity to provide comprehensive nursing care to patients in the comfort of their own homes, fostering independence and promoting optimal health outcomes. This one-on-one healthcare approach allows you to connect and collaborate with individuals on their care, while acting as an advocate for their needs and quality of life. Responsibilities include: Conducting comprehensive assessments of patients' health status and developing individualized care plans in collaboration with the interdisciplinary team Providing skilled nursing care to patients in their homes, including medication management, wound care, and infusion therapy Performing OASIS assessments and documentation accurately and timely to ensure compliance with regulatory requirements Educating patients and their families on disease management, medication administration, and other aspects of health to promote self-care and independence Monitoring patients' progress and response to treatment, adjusting care plans as needed to achieve desired outcomes Providing emotional support and counseling to patients and families coping with chronic illness or end-of-life care In the Home Health setting, you will be a vital link between a patient and their family to resources across the care spectrum.
