Ascension is seeking a dedicated Registered Nurse Patient Navigator to join our evolving healthcare team. Educate patients about health management, including diagnostics, treatment protocols, and preventive health Coordinate care across multiple providers to streamline patient experience and maximize effective treatment Assess patient needs, developing and implementing nursing care plans Monitor patient progress and adjust care plans as needed Advocate for patients' needs and optimize their access to necessary healthcare services Document all patient interactions, treatments, and outcomes reliably Maintain high levels of patient satisfaction by providing timely and accurate information Strong interpersonal and communication skills Proficiency in electronic health record (EHR) systems Ability to work effectively in a team and independently Strong organizational and multitasking skills Comprehensive knowledge of healthcare regulations and pathways Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid Registered Nurse (RN) license Certification in healthcare navigation or patient care coordination is preferred CPR certified
