As the Wound Care/Treatment Nurse provides care and consultation across the continuum for individuals with wound care and skin care management needs. Collaborates with physicians, nurses and other health care providers in research-based treatment modalities for this specialty area. Participates in product selection and equipment management for medical supplies related to cost effective wound care and skin care practice. You will be responsible for:

• 
  
• Responsible for assessment and treatment based on current clinical knowledge and physician orders. Establishes priorities based on resident needs. Develops care plans, and treatments based on orders to facilitate consistent, optimal quality care and positive resident outcomes.
  
• Consistently applies critical thinking in assessment and assembly of resident clinical data for identification of actual and potential care needs.
  
• Reviews the Treatment Administration Record, Nurse Notes, and Physician Orders for completeness of the documentation. Responsible for assessment of continence or incontinence issues, if requested, and suggestions/recommendations for treatment of resident condition to reduce likelihood of possible skin breakdown. Notifies the physician of any resident with two weeks of unchanged status or a deterioration wound or ongoing pressure ulcer.
  
• Responsible for a head to toe assessment at the time of admission, readmission, any significant change in status, responsible for documentation of the stage of the wound on admission and at the time of discovery with weekly notes, responsible for up dating the care plans when the wound develops and when the skin change has resolved, and responsible for completion of the Braden Scale weekly and ongoing.
  
• Notifies the responsible party of any new or changes in skin condition, completes and charts treatments daily as given according to the Treatment Administration Record, ensures the availability of all needed products for skin integrity and reports on use of products monthly as indicated.
  
• Provides the Interdisciplinary Team updates whenever a change occurs in the status of a wound or when the treatment plan is altered, provide to the Quality Committee reports and information on wounds /other skin changes as needed.
  
• Communicates with attending/consultant physician to assure treatment orders /recommendations are clarified and documented for all interventions. Follows LTC facility procedures for documentation of resident condition, corresponding with plan of care, charts, interventions and response, including family dynamics.
  
• Responsible for the assessment and early intervention for residents at risk for skin breakdown, outcomes-oriented management of chronic wounds (i.e. pressure ulcer and lower extremity ulcers), rehabilitative care for residents with ostomies or continent diversions, cost-effective management of chronic percutaneous tubes, draining wounds, and fistulas; and effective management of bowel and bladder incontinence to correct or manage clinical conditions.
  
• Participates in orientation of new personnel by providing constructive feedback and suggestions through education.
